Our office hours are Monday through Friday from 8:30 a.m. to 5:00 p.m. Our physicians see patients via appointments throughout the week, but in many cases patients may be seen immediately.
When calling our office to schedule an appointment please provide your insurance number to our staff. We participate in a variety of insurance programs, including Medicare, but we will need to verify your coverage and receive authorization before your appointment.
We ask all new patients to please bring with them the following:
- Completed patient information forms (a package will be sent or you can download the necessary forms below)
- Rehab Patient Intake Forms
- Neurosurgery Intake Form
- Neurosurgery Medical Justice Form
- Your insurance information
- Your driver’s license (if applicable)
- All x-rays/studies previously performed
- Any other appropriate medical records
We make every effort to see patients promptly and on time so we ask that you arrive at your scheduled time. Sometimes, however, emergencies occur and we may fall behind schedule. During these moments, your patience is appreciated.
If you have an urgent problem, please call our office for instructions. One of our trained physician assistants will take your message and relay your information to your physician. Your call will be returned as soon as possible. One of our physicians is available 24 hours every day.
For true emergencies, it is best to go immediately to the nearest emergency room of the closest hospital. Once there, hospital staff members will contact our physician while the emergency room doctor begins treatment.
Please call our office if you have questions or problems regarding medications or treatment. Our physician assistants are trained to answer your questions, arrange prescription refills, etc.
Please try to foresee the need for medication refills and call for them during normal office hours. At night or weekends, the physician on-call will not have access to your medical records and may not be able to authorize a drug prescription.
No medicine refills will be provided after hours or on weekends.
When calling for a prescription refill, please have the exact description of your medication and your pharmacy telephone number.
Payment for service is due at the time of your appointment. You may pay by personal check, Visa, MasterCard or American Express.
It is very important that we have the correct information from you regarding your insurance plan and coverage. Ultimately, you as the patient are responsible for payment.
If necessary, payment plans can be arranged by our Billing Department.
